Steps for Getting In
1. Complete and return the application for admission and college preparatory course report form to the Office of Admissions and Records together with payment of the $35.00 application fee. The application fee is a one-time fee payment required by the State of North Dakota.
2. Submit an official high school transcript or official GED Report directly to the Office of Admissions and Records. The initial high school transcript should be complete through the end of the junior year and followed by a final transcript upon completion of all high school requirements. An admissions decision will be made after the sixth or seventh semester whenever possible with the understanding that a final academic transcript will be submitted upon graduation. A final high school transcript must show the date of graduation and be signed by an official at the high school. If the final transcript is not received by the third week of the student's attendance, subsequent registrations may not be allowed. All freshman applicants graduating from high school in 1993 and thereafter must verify that core curriculum requirements have been met in order to be admitted to the University. 3. Submit official test results from either the ACT or the SAT. Applicants should take either test at a national testing site during the junior or senior year. Only the ACT is offered to individuals on campus after enrollment. High school counselors or principals should be contacted for information pertaining to registration and score reporting of these tests. Applicants may apply for ACT or SAT exemption status by virtue of age, previous college experience, or because of extenuating circumstances. Mayville State’s ACT school code is 3212. Mayville State’s SAT school code is 6478.
NOTE: Failure to list all colleges, universities, technical, proprietary or other schools attended (beyond high school), whether or not credit was earned, may result in dismissal or loss of credit or both.
High School Graduate Requirements
Graduates of approved North Dakota high schools are admitted to the university upon presentation of an official high school transcript and official ACT or SAT test scores. Graduates of nonapproved high schools must present an official report of the GED. Graduates of high schools in other states will be admitted upon meeting these same requirements. Freshmen applicants with less than a "C" or 2.00 grade point average will be accepted on academic probation. As of 1993, all in-coming freshmen are required to meet specific subject or core curriculum requirements.
The North Dakota University System College Preparatory Core Requirements include the following:

4 units of English.

3 units of Mathematics, Algebra I and above.

3 units of Laboratory Science, at least 1 unit each in 2 or more of the following: Biology, Chemistry, Physics, or Physical Science.

3 units of Social Studies, exclude Consumer Education, Cooperative Marketing, Orientation to Social Science, and Marriage/Family.
The following high school courses are also strongly recommended:

Algebra II (advanced algebra).

2 units of a single classical or modern language, including American Sign Language and Native American Languages.
Applicants should consult their school counselor or the Office of Admissions and Records regarding acceptability of high school credits.
Transfer Admission
In addition to the application for admission, students transferring from another college or university must submit the following:

A complete and official transcript of all college credit earned from each institution attended.

A high school transcript and ACT or SAT test scores only if less than 24 semester or 36 quarter hours of college course work has been completed. If the Office of Admissions and Records has not received such documentation by the third week of the student's attendance, registration may be terminated.
Transfer students must meet the following academic standards for admission to the university:

1.70 GPA at the conclusion of the semester in which the total number of institutional and transfer hours attempted reaches 24 semester hours; and,

2.00 GPA at the conclusion of the semester in which the total number of institutional and transfer hours attempted reaches or exceeds 64 semester hours.
Once admitted, however, each transfer student will be allowed to enroll for at least two semesters, provided they have a 1.00 GPA or above for the first term.
The applications of transfer students who are subject to an existing suspension order by another institution or who fail to meet the above standard for admission will be denied admission and given the right to appeal before the Admissions and Academic Standards Committee. The committee may impose special conditions of admission for transfer students who fail to meet the academic standards. Academic probation, an existing suspension order, or disciplinary action by another school is honored.
Transfer Credit Policy
All credit hours earned at any regionally accredited institution of higher education are accepted in transfer. The application of these hours to the degree requirements of Mayville State is the responsibility of the faculty. Transfer credits are evaluated by the registrar according to Curriculum Committee approval and in consultation with appropriate faculty. Students who are dissatisfied with course equivalency determinations may consult the registrar and directly with the division chair involved, in an attempt to more precisely define equivalencies or identify appropriate substitutions.
Appropriate transfer course work may fulfill a specific general education requirement if that course is no more than one semester hour less than the institution's requirement. If a course does not appear on the transfer evaluation under general education or the major/minor, it will be applied as an elective towards the 128 semester hours required for graduation.
Total hours earned from all transfer institutions are entered on the Mayville State transcript. Transfer courses and grades do not appear on the Mayville State transcript and are not used in the calculations for the grade point average; therefore, only Mayville State course work and grades are reflected on the Mayville State transcript.
Immunization and Physical Forms
For All Students born after 1956: Proof of two immunizations against measles, mumps and rubella (German measles) must be provided and signed by a health care professional or be supplied by a school or by other official documents. The North Dakota State Board of Higher Education has ruled that no one born after 1956 can attend a state-supported college or university without proof of measles, mumps, and rubella immunization. For Athletes Only: A physical form is required and must be current within three months of the date you plan to enroll at Mayville State if you plan to participate in intercollegiate athletics.
