Transcripts
Mayville State University provides transcripts for all currently or previously enrolled students. Requests for transcripts must be in writing, signed, and dated by the individual whose record is being requested. Download and print the Transcript Request Form (PDF).
The following information must be included:

Student’s name (current and former name)

Social Security Number
-
Current Address & Phone Number (so that we may notify you if transcript cannot be sent)
-
Date of Birth
-
Approximate Dates of Enrollment

Name and Address for Transcript Destination
Transcripts sent to colleges or universities, prospective employers, funding agencies, etc are considered “official”. Transcripts sent directly to the student are stamped “Issued Directly to Student” and are considered “unofficial”.
Transcript requests are processed daily. However, requests made at the beginning or end of a term may require an extra day for processing.
A transcript request will not be processed if a student owes money to the college or is in default on a student loan.
